Downstate State Cup Registration
Teams,
Susan Garner has agreed to hold a downstate registration for the State Cup Monday, May 12th at 6:30pm at Magnolia's Restaurant on Cedar Neck Road. You will need to register at Kirkwood on Friday May 16th if you miss this night.
Magnolia's Restaurant closed due to the weather. Registration will instead be held at Susan's house at 48138 Yacht Basin Road. Yacht Basin Road is the next left after Magnolia's Restaurant. Her house is the 13th on the right. Call her cell if you have any difficulty 858-7477.
The following items are required at registration:
- Player Passes. (in alphabetical order)
- Team Official Passes. (coaches, managers, trainer)
- 3 copies of your state roster
- Medical Release Form: One for each player (in alphabetical order)
Susan will use this registration sheet to check your paperwork. Please print so you can be ready
